APPOINTMENTS AND SCHEDULING

All scheduling of office visits is done by the Front Office Reception Desk. Our main office number is (303) 679-8500. For billing or insurance questions, you may call the department directly at (303) 679-8522.

GENERAL INFORMATION

• We ask that you please bring your Insurance Card to every visit, even if your information has not changed. This allows us to verify the information on file and be sure we are billing properly.
• Please be prepared to bring any co-pay that is due at the time of service.
• For self-pay patients, payment is due in full at the time of your visit. A 15% cash discount is offered if your visit is paid in full the same day of service.
• For your convenience, we accept Visa, MasterCard, American Express, Discover, cash, or check with appropriate ID.

BILLING AND INSURANCE

• We accept almost all insurances except we are not Medicaid providers. Please feel free to call our office to verify that we accept your insurance and any other questions you may have regarding eligibility. We ask that you contact your insurance provider to assure that we are in your network.
• Our Billing Office is located on site and there is a billing specialist usually available during regular business hours M-F from 8-5 for any questions and concerns. For billing or insurance questions, you may call the department directly at (303) 679-8522.
