BILLING AND INSURANCE

We accept almost all insurances except we are not Medicaid providers. Please feel free to call our office to verify that we accept your insurance and any other questions you may have regarding eligibility. We ask that you contact your insurance provider to assure that we are in your network.

Our Billing Office is located on site and there is a billing specialist usually available during regular business hours M-F from 8-5 for any questions and concerns. For billing or insurance questions, you may call the department directly at (303) 679-8522.

FAQ

  • What should I bring in with me?

    We ask that you please bring your insurance card to every visit, even if your information has not changed. This allows us to verify the information on file and be sure we are billing properly.

    In addition, we ask that you bring all current medications with dosages and any required co-pay.
  • Do I have to pay a Co-pay?

    If you have a co-pay, it is the contracted agreement that you have with your insurance company and we are required to collect it when you check in, prior to your visit. It is your responsibility to bring any co-pay that is due at the time of service. We can help you determine what your co-pay is or you can contact your insurance company directly for full information.

    For self-pay patients, payment is due in full at the time of your visit. A 15% cash discount is offered if your visit is paid in full the same day of service.
  • What types of payment do you accept?

    For your convenience, we accept Visa, MasterCard, American Express, Discover, cash, or check with appropriate ID.
  • Looking for new insurance for 2017?

    Commercial Plans only Bright Health Care, Friday Health Plans, and Anthem Pathway Plans.
